Use this form only to request a Field Trip Reservation to McCurdy’s Farm. Use the ‘Tab’ key or your mouse to navigate thru the form. Clicking the ‘Enter’ key prematurely will result in the form attempting to be sent and errors will be displayed. Be sure to fill out all the required fields.
